In-House Dental Savings Plan Maplewood

Hilltop Dental Savings Plan is an annual dental membership plan for individuals and/or families without dental insurance. This plan cannot be used in conjunction with any other dental plan or dental insurance. There are no waiting periods, no maximum limitations and no family or individual deductibles. Membership begins from your enrollment date and terminates after 12 consecutive months.
Benefits Include:
- Two preventive cleanings & exams
- Bitewing X-rays once per year
- Full Mouth X-rays once every 3 years
- Two (2) Fluoride Treatments per year
- Additionally, a 20% courtesy discount will be given on all other dental procedures! (excluding Implant and Orthodontic services).
- For lmplant and Orthodontic services, a 15% courtesy discount will be given.
Initial membership is $349.00 and each subsequent dependent/family membership is $299.00. Eligible family members include spouse and dependent children under the age of 19 or age 26 if full time student.
ALL ENROLLMENT FEES ARE DUE AT THE TIME OF SERVICE. NO REFUNDS WILL BE GIVEN ONCE TREATMENT HAS BEGUN.
